Heart worm: if you start at 3 - 6 months old you do not need a blood test, later you have to test to make sure the dog is not already infected.

Do not use the Lepto vaccine unless your vet recommends it in your area, it has been known to cause bad reactions and deaths in a lot of pups.
Puppy vaccines series should begin at 6-8 weeks old, & continue in 3-4 week intervals until at least 16 weeks. After that, it's up to the hospital's vaccine protocol as to how often they are given. Animal law in your area determines by what age rabies vaccines need to begin.
Heartworm prevention is no longer available as a shot; your choices are tablet or chew form or topically with Revolution. She can start as early as 4 weeks (for Interceptor & Sentinel), or 6 weeks (Heartgard & Revolution) without testing. However, if she is over 6 months, they may suggest testing her before starting the medication.
