its when they start getting stuck at the surface and can't (despite desperate fin action) get themselves down to the bottom u want to get worried!

Sounds like it lost the ability to control buoyancy. Buoyancy of a fish is control by the air bladder. Since goldfish has a tendency to overeat, it will cause damage to the air bladder. Some will heal itself while others will suffer permanently. There is nothing you can do to help. Just make sure you don't overfeed goldfish from now on. As long as the goldfish has the ability to get to the food, it will live. You can also reduce the water current in the tank to make it easier for the goldfish to swim around. Strong water current will also 'pin' goldfish into a corner.
A lot of fancy species of goldfish tend to skink to the bottom due to the weight of their bodies, long fins and lack of buoyancy..
I doubt taking it to the vet will help unless you manage to find one of the few vets who specialize in fish. Probably the best thing you can do is remove 50% of the water and replace it with fresh, conditioned water
don't worry about it goldfish are cheep flush it down and get a new one
