Nitrogen cycle process is not a set time in how long things will go. There are quite a few cases where each stage can linger for various reasons, and your testing amounts will help you gauge better where you are in that, however testing for them would want to be ammonia, nitrite, then nitrate in that order, as thats the order they would normally appear in your tank.

Things that will help you keep your ammonia lower is to reduce food amounts to one feeding a day, exactly how much food the fish will consume in 2-3 minutes. Performing your water changes with cleaning in the substrate 2 times a week will also help. Changing out the water at levels of 30-40% is a good way to help keep the ammonia at lower levels that wouldn't reach lethal levels. Staying on the top of the water is usually a good sign of that being ammonia, so do check that first, and also check on your ph. You wouldn't want to shift your PH with fish in there, but a higher ph is good to know if you have it there, because higher ph's make your ammonia that much more toxic.

Check your nitrates,nitrites, and ammonia. It sounds like a chemical prob especially since your in only week 3 of like a 8 week cycle. That's why your fish died.
