could be hith (hole in the head) Oscars are known for geting this disease but then again it's not his head I would go to a pet store and ask if it is they have medications for it and you will have to do frequent water changes. How often do you change the water now and what are your water parameters? But I would keep an eye on him how is his behaviors is he acting normal?
sounds like hole in the head. it is caused by a protozoan infection in his digestive tract and the holes are a sigh of a nutritional deficiency caused by the infection. You should stop feeding live food (usually the cause of the infection) and speak with a specialist about the problem. Aquatronics use to make a medication specific to this type of protozoan infection but i am not sure if they are still around and still make the medication.

As far as I know people still have no clue what really causes it (though there are many theories), but they recommend keeping the tank SPOTLESS and making sure he/she gets good nutrition. A clean tank does seem to help clear it up. Normally, as far as I have heard, other things associated with it would only be secondary infections/parasites that take advantage of it and not the root cause.
