Driftwood and some other substances can also make the water acidy esp if it is a big piece and a small tank.

What size tank is this? Are there any other fish in there?

Anyway, check your PH, ammonia and nitrite levels. If no fish is picking on him then these will help you idnetify the problem. Remember, high ammonia or nitrite likely indicates overfeeding. I suggest this because it likely that if he starts to get better, then worse again the causative factor may still exist.

PS. Make sure to follow directions when using antibiotics as some people use lower doses to 'stretch' the medicine but you may end up with a bacteria resistent to the medicines.

PSS. If the fin root has been damaged - meaning the base of the body where its tail grows from- some of it may not grow back.

I had the same problem with my loach. The whole tail fin roted off the fish and it took about a month for it to grow back completely but my loach died after about a week after. I did every thing you did but nun of that stuff worked for me so you could be looking at another fish soon.
congrats on keeping your Betta a live for this long! from the sound of it your doing a great job taking care of him. But when you add the medication, take out the filter just to be on the safe side. when the fins grow back, they are very delicate. you may also want to try some freshwater aqua salt. Betta's like this weather they are sick or not. but having it in there helps them deal with the shock. you may also want to change the water once to twice a week, if you don't have the filter in.
