Its a bacterial infection called popeye. It is caused by poor water quality. Koi need 1000 gallons PER fish and an addition 100 gallons PER extra Koi. Koi and plecos (aka sucker fish) excrete ALOT of ammonia and in a tank, you need to do alot of partial waterchanges to control it. Never go past 50 percent and make sure the new water is dechlorinated and the same temperature as the aquarium water so you dont shock the fish.

You will need a medication that treats popeye. I believe Maracyn treats it from Mardel.

your koi probably has a bacterial infection called Pop-Eye

you can get some antibiotics at your pet shop or you can try a

salt bath:

get a clean bucket and fill with water that the fish is in now

add 3 handfuls of rock salt (non iodized) or aquarium salt

catch the affected fish and put him in the water, in a minute or so he will faint, then remove him and put him back into the aquarium
