Sand Crack- a crack or fissure in the hoof of a horse, extending from the coronet downward toward the sole, caused by a dryness of horn. Also called quarter crack.
Contracted hoof- Contracted heels are seen principally in the forefeet of light horses. It may be caused by improper shoeing that draws in the quarters, which prevents hoof expansion and adequate frog pressure
Bruised sole- Ok. simply put is a hematoma of the sole. Lots of ways to get one.

A disease is a disease of the foot - and the only one I can think of off hand would be white line disease - a chronic condition that actually has the qualities of a disease - it spreads and can cause the hoof to slough off. Navicular can be classified as a disease, too. Nothing you've listed qualifies as a hoof disease.
