Steve Irwin was swimming in the Barrier Reef off Australia doing some filming. He was swimming above a string ray, when the ray got spooked and attacked him. The ray hit him with his tail just below the rib cage on the left side sending the barb up under his rib cage and into his heart. He pulled the barb out immediately. I'm sure without an autopsy report, the barb put a large hole in his heart and he bled to death. He lost consciousness right away, and was dead by the time they got him back into the boat. What a tragedy and a great loss!
