It looks like a baby chipmunk. First, see if you can take it to a vet or call a wildlife official. We shouldn't interfere with wild animals. If you can't do either, make sure you're wearing gloves when you're handling this guy so you can protect yourself and protect him/her.
Go to a pet store and look for something called a "lab block." Chipmunks aren't pets, but this food is probably the closest thing you can find to supplement it's diet if it is weaned. Soak it in some milk and offer it to the fellow if you can get it calm enough to eat. It looks like it should be weaned by this point. However, if you want to try feeding it milk, look for KMR (kitten milk replacement) at the pet store, buy one of the bottles there if it doesn't come with one, and follow the instruction on the KMR whether it's powder or liquid.
If you're feeding it milk, don't forget to stimulate it to poo/pee after it's fed. Failure to do this may result in constipation because animals young enough might not be able to do it themselves so you have to take the place of it's mother and use a warm, damp cotton to rub it's back end.

If it doesn't like you, give it to your animal shelter. Although, eventually, they'll kill it because no one would want it.
